Health Records and Privacy Policy - Sept 2024

Records about your care are held electronically and may include person identifiable information e.g. name, DOB, address etc., details of contacts our team shares with you and reports about your care. Your records are to ensure we have up to date information regarding your needs and requirements to ensure you receive the best quality of care. Most of the information we process is provided to us directly by you to assist with your assessment/treatment. We only receive personal information indirectly if consent is given. The Physio Clinic @home only use data for professional purposes and it is processed fairly and lawfully and for specific purposes in line with GDPR. Information is not shared with third parties unless explicit consent is given or where we are legally obliged. All records will initially be handwritten but these will then be stored electronically for up to 8 years prior to being confidentially destroyed
